i <br /> Abdelwahed Facility December 31, 2013 <br /> Stockton California Well Destruction Report <br /> Well Destruction Fieldwork <br /> Horizon witnessed Penecore conduct well destruction fieldwork during the period from <br /> October 22 to November 4, 2013. Inspector Mr. John Yoakum of EHD observed daily well <br /> destruction operations. The following wells were destroyed by the pressure grouting <br /> method: monitoring wells MW-4, MW-7 and MW-8. The following wells were destroyed by <br /> the over-drilling method: MW-2, MW-3, MW-6, MW-9 through MW-13, sparge wells SW- <br /> 1, SW-2 and vapor extraction wells VW-I through VW-5. <br /> Pr onto destruction activity, measurements indicated that the total depth in each well casing <br /> was within 14% of the total installed depth. In the pressure grouting method, neat cement <br /> grout was pumped into the well casings via tremie set to within three feet of the total <br /> measured depth of the wells. The grout level was brought to the top of well casing and was <br /> topped off until stabilized. The well materials were drilled to a depth of approximately 3 <br /> feet to a diameter of approximately one foot larger than the original borehole. The bore hole <br /> was deepened to expose approximately 6 inches of the casing. Neat cement grout was <br /> emplaced in the bore hole to within I foot of the surface. <br /> In the over-drilling method, well construction materials were drilled out to the diameter of <br /> the original borehole and the total-installed depth of the well. Neat cement grout was <br /> emplaced in the bore hole to within 3 feet of the surface as the auger string was extracted <br /> and topped off until stabilized. Approximately t/2 foot of bentonite was emplaced over the <br /> grout to create a base for the backfill material. Sand backfill was placed to within one foot <br /> of the ground surface. <br /> At well VW-4, the advancing steel guide leading auger was lost in the bore hole at <br /> approximately 15 feet below surface grade; the remaining 10 feet of well material was not <br /> drilled out. Approval of the shorter over-drill of well VW-4 was issued in the field on 1 j <br /> November 2013. <br /> All well vaults were removed and bore holes were backfilled to flush with the surrounding <br /> surface. The calculated volumes of grout required to destroy the wells and the actual <br /> emplaced grout volumes are summarized in the enclosed Table 1. <br /> Solid well destruction materials and soil cuttings were contained in a roll-off bin, and <br /> equipment rinse water was contained in DOT-approved, 55-gallon drums for temporary on- <br /> Site storage. On 19 November_2013, Belshire removed the non-hazardous solid waste from i <br /> the Site. On 21 November the solid waste was disposed to Forward Landfill (Class II) in <br /> Manteca, California. Horizon transported and disposed the non-hazardous liquid waste to <br /> InStrat in Rio Vista, California on 28 October and 9 December 2013. Copies of the non- i <br /> hazardous waste manifests are enclosed. <br /> Penecore documented the destruction of each well via a Department of Water Resources <br /> (DWR) form 188 Well Completion Report.
