(Horizon) is forwarding the <br /> enclosed well permit applications for the installation of a total of seven wells related to the <br />! subject site. Four on-site wells will be as follows: one groundwater monitoring well (MW- <br /> 8), two vapor wells (VW-1 and VW-2) and one sparge well (SW-1). Three off-site wells <br /> will include: groundwater monitoring wells (MW-5, MW-6 and MW-7). At this time the <br /> City of Stockton Public Works Department is in the process of approving the Revocable <br /> Permit and the Encroachment permit for the work-planned in the City Right-of-way. <br /> Also enclosed are Licensed Contractors Declaration forms completed by Mitchell Drilling <br /> Environmental Corp., site maps for each group of wells and a check for $178 to cover the <br /> application fees for two separate permits for the two locations of the work. The fieldwork is <br /> tentatively scheduled for the week of February 21, 2005.
