You were given thirty (30) days from the date <br />of the letter to respond. <br />This letter is to notify you that the Underground Storage Tank Cleanup Fund (Fund) is proposing <br />to withdraw your LOC and close your claim from the Fund for the following reason: <br />Your last reimbursement request was received by the Fund on May 12, 1994. If you do <br />not submit a reimbursement request or adequate explanation within 30 calendar days, the <br />Fund will proceed with the closure of your claim. <br />If you are not in agreement with this decision, you may request a review of the decision by the <br />Manager of the Underground Storage Tank Cleanup Fund Program within thirty (30) days from <br />the date of this letter. Please send any request for review to: <br />Mr. Dave Deaner, Manager Claim No. 000122 <br />State Water Resources Control Board <br />Division of Clean Water Programs <br />P. 0. Box 944212 <br />Sacramento, CA 94244-2120 <br />If a request for review of this decision is not received within thirty (30) days from the date of this <br />letter, your LOC will be withdrawn and your claim will be closed.
