You were given thirty (30) days from the date of the letter to respond. This letter is to notify you that the Underground Storage Tank Cleanup Fund (Fund) is proposing to withdraw your LOC and close your claim from the Fund for the following reason: Your last reimbursement request was received by the Fund on May 12, 1994. If you do not submit a reimbursement request or adequate explanation within 30 calendar days, the Fund will proceed with the closure of your claim. If you are not in agreement with this decision, you may request a review of the decision by the Manager of the Underground Storage Tank Cleanup Fund Program within thirty (30) days from the date of this letter. Box 944212 Sacramento, CA 94244-2120 If a request for review of this decision is not received within thirty (30) days from the date of this letter, your LOC will be withdrawn and your claim will be closed.
