REQUESTS FOR CLOSURE <br />*#.s <br />Z <br />V <br />CLOSURE DOCUMENTATION <br />The minimum information required in the closure report includes: <br />i MIH 1994 <br />1. Distance to production wells for municipal, domestic, agriculture, <br />industry and other uses within 2000 feet of the site, <br />2. Site maps, to scale, of area impacted showing locations of former <br />and existing tank systems, elevation contours, gradients, and <br />nearby surface waters, buildings, streets, and subsurface utilities, <br />3. High and low ground water levels (below ground surface), <br />4. Tabulated results of all sampling and analyses, <br />5. Vertical and lateral concentration contours of contaminants found <br />initially, and those remaining in soil and ground water, both on <br />and off site, <br />6. bass balance calculation of the substance treated versus that <br />remaining, <br />7. Technology used to clean the site; if Best Available Technology <br />(BAT) was not used, explain why, <br />8. Zone of influence calculated for the subsurface remediation system <br />and the zone of capture attained for the soil and ground water <br />remediation system, <br />9. Reasons why "background" was/is unattainable using BAT, and <br />10. Rationale why conditions remaining at the site will not adversely <br />impact water quality, health, or other beneficial uses. <br />Additional information submitted, such as risk assessments or fate and transport <br />modeling, must include the assumptions used. <br />Upon review, if Board staff determines that the Closure Report substantiates the <br />request for closure, the remedial and monitoring activities may cease. Upon <br />concurring with the closure request, Board staff will prepare a letter which <br />summarizes the site conditions and includes the following: <br />'Based on the information provided, Board staff has determined [and <br />the Local Implementing Agency concurs,] that appropriate response <br />actions have been completed, that acceptable remedial practices were <br />implemented, and that, at this time, no further investigation, remedial <br />or removal action, or monitoring is required at [Facility name and <br />address]. <br />
