Box 388 <br /> Stockton, California 95201-0388 <br /> Application for Boring Permits <br /> McMullin Dehydrator Station(SBE-135-39-43-1) <br /> Airport Way 0.3 miles South Perrin Road <br /> Manteca, California <br /> Dear Ms. Lagorio: <br /> Harding Lawson Associates(HLA), on behalf of Pacific Gas and Electric (PG&E),is performing Soil and <br /> Groundwater Preliminary Environmental Assessments at 69 PG&E Gas Dehydrator Stations in Northern <br /> California. This work is being performed under the regulatory oversight of the Regional Water Quality <br /> Control Board-Central Valley Region(RWQCB). RWQCB oversight is being provided by Ms. Wendy <br /> Cohen and Mr.Joel Weiss. The Work Plan and Work Plan Addendum for the first eleven sites has been <br /> approved and a copy of the RWQCB approval letter is attached. Based on our discussion, a description of <br /> the field procedures and methods is attached to facilitate your processing of the permits. <br /> HLA submits a completed application for 4 soil/hydropunch borings(1 hydropunch per boring) at the <br /> McMullin Dehydrator Site (Location Map) at the above location. Enclosed are a check for$323.00 <br /> covering the application and Site Mitigation Program fees, a completed permit application with attached <br /> site plan, a Masterfile Record Information Form and General Program File Form, a California Licensed <br /> Contractor Questionnaire, a copy of the Well Drillers License with expiration date, a copy of the Workers <br /> Compensation certificate of coverage, and a copy of the well driller's authorization letter to sign permits. <br /> The field work is tentatively scheduled for the third week of February, with Gregg Drilling and Testing <br /> Inc., a C-57-licensed(#485165) well driller from Martinez, California. The work will commence at about <br /> 8 AM and will probably take most of the two days to complete. Grout inspections will be scheduled at <br /> least 48 hours prior to the requested inspection date. Your ongoing cooperation on this project is greatly <br /> appreciated by HLA and PG&E.
