<br />1) The PERMIT TO OPERATE will becc-ma- void if ANPUAL PERMIT Fees ark' SERVICE Fees are not paid and/or the UST system(s) f_.i:e

<br />2) The PERMIT TO OPERATE is granted t_, the TA�'� OWER who accepts respo sibiiity for operating and monitoring the UST systew
<br />.according to State underground storage tank• laws and regulations as well as any conditions established by 'San Joaquin County.
<br />:3) The T"'*" C9ERATOR(S), if different from the tank owner, shall owate and �+r;itor the lKiT system according to t•he WRITTEN
<br />OPERATING AGREEMENT required under Section 2.293, Chapter 5.7, '}ivision 20, California Healt, and Safety Code.
<br />4) The TANK W74ER shall notify the Environmental Health Division of any Proposed change in operation or ownership of the U T
