<br />:) Th:_ PERMIT TO OPERATE will become void if AMIAL PERMIT Fees and SERVICE Fees are not. paid and/or the ?ST systems) fails
<br />to remain in compliance with the PERMIT UNDITIONS.
<br />2) The PERMIT TO OVERATE is granted to the TAtdk OWNER who accepts responsibility f; --!r operatir!q and rsr.nitoring the UST system
<br />according to State underground storage tank laws and regulations as well as any conditions established by "Rn Joaquin County.
<br />3) The TAW OPERATOR(S), if different from the tank owner, shall operate and monitor the UST system, according to the WRITTEN
<br />OPERATING AGREEMIEMIT required under Section 25:.x+3, Chapter 6.71 Division 20, California Health and Safety Code.
<br />41 The TAW OWNER shall notify the Errvirorwvntal Health Division of any proposed change in operation ofi ownership of the Ui'T
<br />system.
<br />5) Upon any changein e;.aipn!ent, design! or operation of this facility, the PER -11 TO OPERATE will -e reviewed by the
<br />Environmental Health Division.
<br />6) A construction or removal permit is r'equir'ed from the Environmental Health Division Prior to any r'errF:val or
