13.0 Certified Industrial Hygienist <br />Cecilia Stoddard will be the CIH that will be over seeing the safety of <br />1�( <br />the workers that will be entering the tank. She will be responsible for the <br />LEL levels in theA tank during the process of the work. The CIH will be in <br />C�✓'�I <br />attendance for the duration of the project. Her credentials and qualification <br />are being sent to PHS/EHD via Federal Express. The CIH has total authorit <br />�l <br />over the entry of the tank. The CIH must submit an entry permit that meets <br />the California Code of Regulations Title 8, Section 5157.This permit must <br />accompany the Health and safety plan at all times during the operation. <br />13.1 Gas Station Operations <br />All gas sells will halt during the inspection and repair process. Once <br />the tank has been re -certified back to UL requirements. This certificate must <br />be submitted to the PHS/EHD and the station may proceed with their gas <br />sales. <br />14.0 Professional Engineer <br />Joe Plecnik will be the PE that will be entering the tank along with <br />Containment Solutions. Joe meets all the requirements of Section 25281 <br />Chapter 6.7 of the California Health and Safety Plan. All his credentials and <br />qualifications will accompany this Health and Safety Plan. Joe will be doing <br />the final inspection of the tank. <br />15.0 Tank Warranty <br />Containment Solutions will be doing the repairs that Joe Plecnik <br />concludes needs to be done. The repairs that will be done will be covered <br />under the warranty of the tank. The tank manufacturer will offer this <br />warranty and certify that the tank meets all Underwriters Laboratory Listing <br />requirements. <br />
