STAYS AT THE ENTRANCES WHENEVER ENTRANTS ARE INSIDE. <br />2. MAINTAINS ABOVE ENTRANT LOG. PREVENTS UNATUHORIZED ENTRIES. <br />3. MAINTAINS COMMUNICATION WITH ENTRANTS. <br />4. CHECKS AIR TESTING DEVICE READINGS. <br />5. KNOWS POSSIBLE HAZARDS OF SPACE & OVEREXPOSURE SYMPTOMS. <br />6. ORDERS ENTRANTS OUT IF HAZARDS/PROBLEMS/PROHIBITED CONDITIONS DEVELOP. <br />7. KNOWS EMERGENCY PROCEDURES (NEVER ATTEPMTS TO ENTER). <br />8. PERFORMS NO DUTIES THAT 1NTERFER WITH ATTENDANT DUTIES. <br />Attendant acknowledgment <br />DESCRIBE PROBLEMS DURING ENTRY & RESOLUTION: <br />GENERAL EMERGENCY PROCEDURES: <br />1. PREVENT THE EMERGENCY THROUGH PROPER PLANNING & CONTROLS. <br />2. DO NOT ATTEMPT TO ENTER A CONFINED SPACE FOR RESCUE. <br />3. KNOW PHONE LOCATIONS (SITE EMERGENCY PHONE NO. IS "911"). <br />4. USE NON -ENTRY RESCUE METHODS (BODY HARNESS WITH LINES). <br />5. REMAIN OUTSIDE THE PERMIT SPACE, IF IT IS SAFE TO DO SO, UNTIL RELIEVED BY ANOTHER <br />ATTENDANT OR THE PERMIT IS CANCELLED. <br />CONFINED SPACE: <br />Is large enough and so configured that an employee can physically enter & do assigned work AND has limited or <br />restricted means of entry or exit AND is not designed for continuos employee occupancy. <br />PERMIT REQUIRED CONFINED SPACE: <br />A confined space that has one or more of the following characteristics: <br />1. Contains or has the potential to contain a hazardous atmosphere (bad air). <br />2. Contains a material that has the potential for engulfing an entrant. <br />3. Has a sloping wall/floor design which could trap/asphyxiate a person/ <br />4. Contains any other recognized serious safety or health hazard. <br />COMMON CONFINED SPACE HAZARDS: <br />Hazardous atmospheres, residual chemicals, slick surfaces, energies needing lockout, materials in adjacent pipes, <br />steam, temperature extremes, high noise levels, bad lighting, engulfment, electrical, difficult access, traffic hazards, <br />adjacent processes with potential hazards (ignition sources, hazardous chemicals, etc.). <br />
