See attached "Continuation — Official Inspection Report" for violation(s) not listed above. <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR -Notice to Comply) <br />UST Inspection: Routine & Annual Monitoring System Certification <br />204 & 206. The designated operator failed to verify that all employees have been trained during the <br />monthly inspection. Last designated operator training was conducted 06/27/2011. It is 6 months passed <br />due. During the monthly inspection, the designated operator shall verify that all facility employees have <br />been trained. Ensure that designated operators performing monthly inspections at this facility are <br />including all of the required information on the reports. <br />The designated operator employee training for everyone was not performed in a timely manner annually <br />and was supposed to be conducted 06/2012. The designated operator shall train facility employees for <br />which he or she is responsible in the proper operation and maintenance of the UST system once every <br />12 months. The training shall include, but is not limited to: <br />with the facility's best management practices <br />1. Operation of the UST system in a manner consistent <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and <br />submit a copy of the training records to the EHD. <br />Submit to SJC EHD within by the end of the month (01/31/2013): <br />1. one copy of the complete "Return to Compliance Certification" form <br />2. a copy of Designated Operator employee training record <br />Note: CCR, Title 23, Section 2644.1(a)(5). ELD test results must be submitted to State Water Board <br />UST Program and SJC EHD within 60 days after test performed. An Enhanced Leak Detection (ELD) <br />test was scheduled for 11/08/12. There are Praxair borings on tank pad.
