RETURN TO COMPLIANCE CERTIFICATION <br />Item # 123 <br />1 had an impression that permit I have for sump repairs will cover this <br />replacement of same to same part. However Mr. Wayne Henderson of HMC had <br />mentioned about to file an amendment to the original permit, but it slips off my <br />mind. Will be care full in future. The amendment is attached for your approval <br />please. <br />Item # 306 <br />This might be an oversight during the last years test by the technician. <br />There was no work done in this sump. Will be careful in future. <br />Item # 317 <br />(a) Mr. Wayne Henderson of HMC repaired the sump in June 2014. After <br />repairs his certified testing technician was not there to test the leak, Mr. <br />Henderson perform the test (in my knowledge he is not certified for that) that <br />could be the reason or may be his equipments were not functioning properly or <br />may not be properly calibrated. Possibly the sump was tight at that time. After <br />that I called Mr. Henderson so many times but no response. <br />(b) Than I called B.Z. Service Co. In August 2014 when B.Z. Service <br />Company performed the vacuum test the sump was tight that time, which can be <br />evidenced from there report (copy attached), that they filled the sump with water <br />to level above all the penetrations, sealed the top air tight with a see through <br />plaxi glass and vacuum the sump with a machine and continue vacuuming for <br />more than 2 hours, but could not see a single air bubble coming from any of the <br />penetration or any part of the sump. Which clearly shows that at that time the <br />sump was air tight, and all the penetrations were properly sealed. <br />Also in their report they have two conflicting statements. (1) On 7/18/14 <br />they said that no obvious leak but (2) On 8/12/14 the same technician saying that <br />the electrical conduit is wide open, and blaming the HMC that Mr. Henderson's <br />work was not done properly. <br />Instead of saying that the sump is tight they told me that it is a big problem <br />as they could not locate the leak, they might have to break the concrete around <br />the sump to locate the leak from outside and they will hire another contractor <br />and work with that contractor. <br />
