The permit has been approved with the following <br />conditions: <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Thuy Tran at (209) 468-8257 to arrange the following inspection appointments <br />(a 48-hour notice is required for inspection appointments). Please be aware that <br />inspections for emergency work must be scheduled within five days of permit approval. <br />a. Sensor functionality: Functionality of the monitoring system must be verified to <br />ensure that monitoring has been restored to meet the requirements of Title 23 <br />California Code of Regulations and the Health & Safety Code. <br />b. Secondary containment testing on 91 STP sump. <br />4. If concrete will be broken near the premium sump, a secondary containment test must <br />be performed on the 91 UST once concrete is poured in accordance with the <br />manufacturer's guidelines. <br />5. Secondary containment testing for the 91 STP is required six months after installation. <br />Provide 48 hours notification to the EHD when this test is scheduled. <br />6. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />
