SUMMARY OF VIOLATION CLASS I,CLASS II,or MINOR-Notice to om I This is a Routine and Complaint Hazardous Waste inspection. he following are Repeat Class II violations observed during this inspection: 108, 122/125, 128 01, 404, 605, fest, 812, afro e o i Ise 3 �'1 108. (Repeat) Sandblast waste from the outdoor sanddaster unit, which had been previously observed in 2011 Routine inspection, still requires hazardous was a determination. A sample has been collected by the EHD during this inspection and a split sample offered to generator. Generator shall make hazardous waste determination of the waste and manage twaste accordingly. Immediately make a hazardous waste determination of the above wastes and anage each accordingly, or dispose of the waste as hazardous waste in accordance to California ode of Regulations (CCR) Title 22. In addition, hazardous waste properties and size /manifesting l particle must be determined before they can be recycled as scrap metal. Hazardous metal p100 mi rons or smaller cannot be recycled but must be handled as hazardous waste. If the wdeter ined to be a non-hazardous, please submit evidence (i.e. third party laboratory testing respp ing such a determination to EHD. Submit a statement and supporting documentation in how this waste was managed by 11/4/2013. 122 & 125. (Repeat) Copiezardous waste disposal records for 2011 were not found on site. Hazardous waste generatol retain copies of all manifests signed off by the disposal facility and all receipts used in a consolidanifesting procedure on site for three years and have them readily available for review. Imme locate a copy of all manifests and receipts for the last three years, maintain them on site, a7reauested submit copies of these records, including the missing disposal receipts between 2009 and 201 the October 2011 inspection to the EHD by 11/4/2013. ALL EHD STAFF TIME AS CIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
