(C) If reportable or if assistance is required, notify the local administering <br />agency, city or county emergency agency designated in the Contingency <br />Plan if a release or threatened release that may cause present or potential <br />harm to human health, the environment or property exists or if evacuation <br />of surrounding community may be necessary.. <br />(D) Immediately notify the local community hazardous materials agency (209- <br />468-3969) or CUPA (209-468-3420) and notify the state Office of <br />Emergency Services (1-800-852-7550) in the above situation. <br />(E) Notify the National Response Center (1-800-424-8802), if applicable, <br />(based on a release in excess of 100 pounds of RCRA hazardous waste or <br />the reportable quantity (RQ) for the material, if less). <br />(F) In all such notifications, the EC or designee shall provide the following <br />information: <br />(i) Nature of the incident (spill, explosion, fire, etc.) <br />(ii) Location of the incident (name and address of facility) <br />(iii) Time/date incident occurred <br />(iv) Name of hazardous material(s) involved <br />(v) Approximate quantity involved <br />(vi) Safe location on or near facility where emergency responders can <br />meet company Emergency Coordinator <br />(vii) Number of injuries and cause of injuries <br />(viii) Whether facility is being evacuated <br />(ix) Advice concerning whether surrounding area should be evacuated <br />(x) Whether individuals have been contaminated <br />(xi) Potential hazards to human health or the environment <br />(G) Determine the necessity of use of facility personnel specially trained to <br />clean up large spills (personnel designated and trained as emergency <br />responders) or the need to notify and await off-site responders. <br />(FI) Notify additional off-site responders identified in the Contingency Plan as <br />appropriate. <br />(1) Assist off-site responders upon arrival at the facility. <br />(J) Submit a report to the Department of Toxic Substances Control within 24 <br />hours if the release exceeds one pound from a tank system or within 15 <br />days of the incident if the Contingency Plan was implemented regardless of <br />the volume of the release. <br />10 <br />
