EMERGENCY REPORT SAN JAOQUIN COUNTY <br /> ENVIRONMENTAL HEALTH DEPARTMENT <br /> 1) NAME,ADDRESS,AND TELEPHONE NUMBER OF THE <br /> OPERATOR <br /> • California Military Department <br /> 9800 Goethe Road <br /> Sacramento, California 95827 <br /> 916-854-3691 <br /> 2) NAME,ADDRESS,AND TELEPHONE NUMBER OF THE <br /> FACILITY <br /> • Organizational Maintenance Shop#24, Stockton <br /> 8010 South Airport Way, Stockton, California 95206-3997 <br /> 209-983-5485 <br /> 3) DATE,TIME,AND TYPE OF INCIDENT <br /> • March 18, 2003 <br /> Approximately 4:30 P.M. <br /> Fuel spill <br /> 4) NAME AND QUANTITY OF MATERIAL REMOVED <br /> • Diesel fuel <br /> Approximately 45 gallons of diesel fuel <br /> 5) THE EXTENT OF INJURIES,IF ANY <br /> • There were no injuries as a result of the incident or the cleanup of <br /> the spill. <br /> 6) AN ASSESSMENT OF ACTUAL OR POTENTIAL HAZARDS TO <br /> HUMAN HEALTH OR THE ENVIRONMENT <br /> • Because of the nature of the diesel fuel (flash point approximately <br /> 125 degrees Fahrenheit-flammable) there was the potential for <br /> fire. <br /> 7) ESTIMATE QUANTITY AND DISPOSITION OF RECOVERED <br /> MATERIAL THAT RESULTED FROM THE INCIDENT <br /> • The spill occurred on a paved area at the facility covering an <br /> area approximately 20 feet by 40 feet. <br /> • The fuel was contained and cleaned up with spill response <br /> materials and deposited into properly labeled Department of <br /> Transportation approved containers. <br />
