This facility does not have a valid EPA ID number to manage hazardous waste. A hazardous waste <br />generator shall not treat, store, dispose of, transport or offer for transportation, hazardous waste without <br />an EPA ID number. Immediately obtain an EPA ID number to manage hazardous waste and submit <br />evidence to the EHD by 4-3-11. A form has been provided. Minor -Notice to Comply. <br />11. Dark staining was observed on the soil next to the 55 drum of oil and the 500 gallon diesel above <br />ground storage tank. Facilities shall be maintained and operated to minimize the possibility of a fire, <br />explosion, or release of hazardous waste to air, soil, or surface water which could threaten human health <br />or the environment. Immediately remove the oil and diesel contaminated soil and manage according to <br />Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining <br />how this waste was managed by 4-3-11. Minor -Notice to Comply. <br />23. Bills of lading for the recycling of used oil filters for the last three years were not found on site. A <br />copy of each bill of lading must be kept on site for at least three years. Immediately locate a copy of all <br />bills of lading for used oil filters for the last three years, maintain them on site, and submit copies to the <br />EHD by 4-3-11. Minor -Notice to Comply. <br />60 & 63. Copies of hazardous waste disposal records for 2008 and 2010 were not found on site. <br />Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all <br />receipts used in a consolidated manifesting procedure on site for three years and have them readily <br />available for review. Immediately locate a copy of all manifests and receipts for the last three years, <br />maintain them on site, and submit copies to the EHD by 4-3-11. Minor -Notice to Comply. <br />64. Copies of manifest or bill of lading for spent lead acid batteries for the last three years were not <br />found on site. A copy of each manifest or bill of lading must be kept on site for at least three years. <br />Immediately locate a copy of all bills of lading for spent lead acid batteries for the last three years, <br />maintain them on site, and submit copies to the EHD by 4-3-11.
