• CONTINUATION FORM <br /> OFFICIAL INSPECTION REPORT <br /> Facility Address: 1011 N. Broadway, Stockton Page 3 of 8 <br /> Program: Hazardous Waste <br /> Time of Inspection: February 18, 2010 <br /> Date of Completed Inspection Report: March 9, 2010 <br /> This is a re-inspection billable at current Environmental Health Department <br /> (EHD) rate of $115 per hour. A letter dated November 10, 2009 was mailed to <br /> the facility informing the generator that EHD has not received the Return to <br /> Compliance and supporting documents stated in the Hazardous Waste (HW) <br /> Generator Program Official Inspection Report (OIR) dated May 28, 2009. This <br /> re-inspection is conducted because this Return to Compliance information has <br /> not been submitted. <br /> Summary of Violation, Class II: 7, 8, 11, 20, 27, 30, 50, 64, 79, 83, 84 <br /> (All violations are repeat violations except 8 and 64) <br /> 7. Observed multiple bottles of petroleum samples abandoned inside the <br /> former Golden Lube building. (Photos #1, #1 A) <br /> Generators shall determine if a waste is a HW and manage the waste <br /> accordingly. <br /> Immediately make a HW determine of the above wastes and manage <br /> them accordingly, or, dispose them as HW in accordance to Title 22. <br /> Submit evidence of correction to EHD by April 10, 2010. <br /> 8. EPA ID # is currently inactive at time of this inspection. <br /> A current active EPA ID# is required for storage and disposal of HW. <br /> Immediately obtain a current active EPA ID# and submit evidence of <br /> correction to EHD by April 10, 2010. <br /> 11. Observed soil impacted by HW release from used oil containers in the <br /> area south of former Valley Shingle at time of this inspection. <br /> Facility shall be maintained and operated to minimize the release of <br /> HW or HW constituents to air, soil or surface water which could <br /> threaten the environment. <br /> Immediately clean up and manage the impacted soil in accordance to <br /> Title 22. Submit evidence of correction to EHD by April 10, 2010. <br /> (See Photos #10, #11) <br /> 20. No HW labels or incomplete HW labels on the following HW containers <br /> at time of this inspection: <br /> a. Three 55-gallon drums of spent HW absorbents in former Valley <br /> Shingle (Photos #3, 4) <br />
