qU Dignity Health. <br /> (Iv St.Joseph's Medical Center <br /> Page 4 of I I <br /> b. Within the Biological Safety Cabinet place the bulk chemotherapy waste in a <br /> zip-lock bag. <br /> c. Then place the bag in a disposable, bulk chemotherapy container labeled <br /> "Hazardous Chemical Waste" and "Incineration only'. <br /> d. Nitrogen Mustard will be inactivated with Sodium Thiosulfate prior to disposal. <br /> e. Record amount discarded on the Sterile Chemotherapy Compounding Log. <br /> f. Retain a copy of the list in the pharmacy for 3 years. <br /> g. When a container is 75% full, call Materials Management and request special <br /> bulk chemotherapy waste pick- up Materials Management staff will deliver the <br /> container to the Hazardous Waste Storage area to be disposed of by a <br /> licensed hazardous waste company. <br /> h. See Materials Management policy and procedure for disposal outside of the <br /> facility. <br /> 3. RCRA (Federal) hazardous waste pharmaceuticals must be placed in a black <br /> container marked "RCRA Hazardous Waste." A completed yellow hazardous <br /> waste label must be attached. Call EVS and request that the waste be taken to <br /> the Hazardous Waste Storage area. RCRA waste is separated into two <br /> separate bins: aerosol and non-aerosol RCRA waste. Label each bin with the <br /> date started and the date to be removed, 11 months from start date. <br /> (See Attachment A for RCRA Pharmaceutical Waste Alphabetical List) <br /> 4. Non-RCRA pharmaceutical waste that cannot be disposed of in the sewage <br /> system or solid wastage system in California (also known as California "only' <br /> hazardous waste) must be placed in a Pharmaceutical Waste container and be <br /> labeled "Incineration Only" on the lid and sides of the container. Request EVS to <br /> take the waste to the Medical Waste Storage area. <br /> V. Education and Training <br /> A. Staff training on appropriate handling of pharmaceutical waste will be done during <br /> orientation of new clinical staff. <br /> B. Compliance with appropriate waste procedures will be audited weekly during <br /> environmental rounds. A schematic diagram of Waste Handling will be available to <br /> staff to assist them in differentiating appropriate waste stream management. (See <br /> Attachment C) <br />
