The emergency response plan and map of all emergency equipment is now posted next to <br />the phone. See appendix A at the end of this document <br />Item #110, 111, and 113 Failure to keep signed manifests for the designated facility for the previous 3 <br />years. — We have retrieved the manifests that have been generated in the last 3 yrs. In 2016 we did not <br />submit a request for pickup of Hazardous materials and have no records of any pick ups made that year. <br />All other manifests have been included in this document See Appendix B. <br />Item # 403 Failure to keep Hazardous waste containers closed except when adding or removing waste — <br />We have immediately fixed this situation and we will not allow any containers to have any missing lids. <br />See Appendix C <br />Item #406 - Contaminated container not marked with date emptied or managed within 1 yr of being <br />emptied. - All containers on site that have no tags have been tagged properly and dated. See <br />Appendix D <br />Item #601- Stored Hazardous waste on site for more than 180 days. — All Stored Hazardouse waste <br />that has no date on containers has been emptied through a licensed haz waste hauler. See Appendix E <br />for manifests. ( some manifests have not been signed by the receiving location. The signatures will be <br />complete by 10/26/2018) <br />Item #605 — Failed to completely label Portable tanks of Haz Waste - All Haz waste storage containers <br />have been properly labeled. See Appendix D <br />
