The issues with responses follow: <br /> Issue#1: <br /> Immediately begin maintaining alarm logs and keep readily available for review. <br /> Response: <br /> Re-training of store employees to maintain alarm logs were conducted by Rudy Tovar,the <br /> Market Manager for the location. The former Market Manager,Terrence Nutt, has been demoted. <br /> The store manager has been counseled on the requirement to maintain complete alarm logs per <br /> the CRR. Rudy Tovar, also the assigned Designated Operator will review these logs during their <br /> monthly inspection to insure the logs completed and maintained as required. Alarm logs are now <br /> being kept and are readily available for review. <br /> Issue#2: <br /> Designated Operator inspections were not found for the months of October 2008,January 2009, <br /> and April 2009. <br /> Response: <br /> Enclosed is the missing October 2008 Designated Operator inspection report. Unfortunately the <br /> January 2009 and April 2009. The former Designated Operator(and Market Manager),Terrence <br /> Nutt,has been demoted. Rudy Tovar is now the assigned Designated Operator. <br />
