4) Steps taken to reduce, eliminate, and prevent recurrence of the <br />release. <br />c. A written report must be submitted to the California Department of <br />Health: Services (DHS) by the PG&E Environmental Service Department <br />within 15 days after an incident that posed a hazard or potential <br />hazard to human health and safety, property, or to the environment. <br />Contact your Regional or Departmental Environmental Coordinator for <br />assistance in preparing this report. <br />The report should include the following information: <br />1) Name, address, and telephone number of facility manager. <br />2; Name, address, and telephone number of facility. <br />3) Date, time, and type of incident (e.g., fire, explosion, spill, <br />etc.) <br />4) Name and quantity of waste(s) involved. <br />5) The extent of injuries, if any. <br />6) An assessment of actual or potential hazards to human health or <br />the environment, where applicable. <br />7) E'timated quantity and disposition of recovered wastes and cleanup <br />naterial. <br />5-29 <br />
