Attachment 5 <br /> Emergency Response Procedure: Hazard Mitigation/Prevention/Abatement [19 CCR§2731(e)] <br /> Briefly describe your business standard operating procedures in the event of a release or threatened release of hazardous <br /> materials: <br /> 1. Evaluate hazards. <br /> 2. If safe, take care of injured. <br /> 3. Call 911 or specific emergency responder if the situation appears to be beyond your ability to resolve immediately. <br /> 4. Don't attempt any action that is beyond the scope of your knowledge, training or experience. <br /> 5. If necessary to evacuate the facility, follow evacuation routes and procedures. <br /> 6. Notify internal emergency coordinator(site emergency contact)and/or the assigned Environmental Field Specialist. <br /> 7. If necessary, arrange for hazardous materials/emergency response contractor to respond and/or clean up spill. <br /> 8. Assure that appropriate agency notifications are made (after situation is stabilized and it is safe to do so). <br /> 1. PREVENTION (prevent the hazard)— Describe what is done to prevent a release or threatened release of hazardous <br /> materials (e.g. discussion of safety and storage procedures). <br /> a. Complete required routine inspections of facility equipment, piping, tanks, shut-off valves and emergency response <br /> equipment. <br /> b. Perform required maintenance of systems identified in routine inspections. <br /> c. Provide secondary containment where required to prevent leakage or spills. <br /> d. Maintain secondary containment(make sure water is removed, leaks and cracks are repaired). <br /> e. Check packaging and lids of hazardous materials containers during routine inspections. <br /> f. Promptly clean up small spills and properly dispose of clean up materials. <br /> g. Segregate incompatible materials. <br /> h. Keep hazardous materials stored in appropriate locations (designated lockers, sheds, etc.). <br /> L Assure that security fencing, locks and gates are in good repair and being used properly. <br /> j. Keep evacuation map and emergency response procedures prominently posted. <br /> 2. MITIGATION (reduce the hazard)—What is your immediate response to a leak, spill, fire, explosion, or airborne release <br /> (e.g. absorbent on spills, use of fire extinguisher, closing of valves, etc.). <br /> a. Evaluate hazards. <br /> b. If safe, take care of injured. <br /> c. Call 911 or specific emergency responder if the situation appears to be beyond your ability to resolve immediately. <br /> d. If safe, attempt to put out a fire using appropriate fire extinguisher. <br /> e. If safe, take steps to prevent leak from worsening or moving towards a waterway or storm drain (close valves, place <br /> sand bags or containment booms). <br /> f. If necessary to evacuate the facility, follow evacuation routes and procedures. <br /> g. If there is potential for the leak or spill to affect people or the environment near the facility, work with incident <br /> commander to notify/evacuate people/animals from the area as appropriate. <br /> h. Notify internal emergency coordinator(site emergency contact)and/or the assigned Environmental Field Specialist. <br /> i. If necessary, arrange for hazardous materials/emergency response contractor to respond and/or clean up spill. <br /> j. Assure that appropriate agency notifications are made (after situation is stabilized and it is safe to do so). <br /> 3. ABATEMENT (remove the hazard)—Describe what you would do to remove any released material (e.g. clean up <br /> company, absorb spill and containerize). <br /> a. Small leaks and spills should be cleaned up using appropriate absorbent material. <br /> b. Remove/repair any leaking pipes, tanks, containment or packaging. <br /> c. Contaminated materials from clean up efforts must be disposed of properly (containerized as hazardous waste and/or <br /> removed by qualified hazardous waste contractor). <br /> d. For large spills (over one gallon, could enter the storm drain or contaminate a wetland, stream, lake or other water <br /> body), immediately call a qualified hazardous materials spill response contractor(work with EFS to access PG&E's list <br /> of qualified hazardous materials spill response contractors). <br />
