HSC 25201(a)Stored Hazardous waste on side longer than 90 days without a permit or <br /> authorization and CCR 66262.34(f)failed to completely label containers or tanks of <br /> hazardous waste <br /> The gray bins are utilized for the defective program. When a potentially hazardous products is <br /> damaged beyond sale, it is placed in the appropriate tote for disposal by your contracted vendor. <br /> The store is making sure that the label is always completed correctly. The name of the store is <br /> number 562. <br /> StrongPak has visited this location on June 15, 2011 and November 23, 2011. No defective <br /> products had been generated, therefore none were picked up. The last pick up that generated a <br /> manifest was on November 19, 2010. Please see exhibit C. <br /> CCR 66266.130(c) Failed to: (1) Drain of free-flowing oil, (2) Reclaim Metal (3)close <br /> container with initial date of accumulation, label "drained used oil filters", (4) store<1 <br /> tol<1 year, (5) Retain bill of lading for filters for 3 years. <br /> The oil filter drum label is provided by the hauler. They store only has to note the accumulation <br /> date. I have discussed with All Phase to assure that the oil filter drum is properly labeled when <br /> dropped off. Discussed with Service Manger to assure that the accumulation date is always filled <br /> in. Exhibit D shows the label provided by All Phase. Also included in exhibit B is a picture of the <br /> drum labeled. <br /> In addition, we have also revised the daily inspections of hazardous waste Tank log to include a <br /> column for checking if he oil filter drum is covered properly. Please see exhibit E. <br /> CCR 66265.192 Failed to-comply with new HW tank system assessment requirements <br /> Pep Boys is aware of the hazardous waste tank system assessment requirements and are <br /> working diligently to get all the store in the state of California completed. Salmon & Schroeder is <br /> the consultant completing the required tank assessment. During the week of January gth the <br /> aboveground tanks were evaluated and found to be in good working condition, but that they <br /> lacked seismic restraints. The restraints are due to be installed by February 17, 2011. Once the <br /> restraints are installed and proper installation is verified, the certification report is forward to the <br /> engineering shop. It's taking 34 weeks for the final report to be issued. The certification process <br /> should be completed by March. <br /> I will send you a picture once I receive confirmation the restraints have been installed. <br /> CCR 66265.195(a) Failed to conduct daily inspections of HW Tanks- <br /> The store had not been completing the correct daily hazardous waste tank inspection. Please <br /> see exhibit E, which verifies that the store has begun the new daily hazardous waste tank <br /> inspection. <br /> CCR 66265.52 Contingency Plan Incomplete and CCR 66265.55 No Emergency Coordinator <br /> The emergency response procedures have been revised and reviewed with store management. <br /> Pease review exhibit F. <br /> During the new training revamping, we are also providing all stores at laminated emergency <br /> procedure guide to all stores. Exhibit G is a sample of what will be provided. They are being <br /> generated for this site on Monday, February 13, 2012.
