San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone:(209)468-3420 Fax:(209)468-3433 Web:www.siaov.ora/ehd <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> TNT AUTOMOTIVE & EXHAUST REPAIR 308 N CHEROKEE LN, LODI November 02, 2016 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS ll,or MINOR-Notice to Comply) <br /> Item• Remarks <br /> 114 HSC 25160.2(b)(4)Consolidated manifest receipts failed to contain complete information. <br /> The consolidated manifest receipt for the disposal of hazardous waste had the old business name and state ID <br /> number that was under different ownership.All copies of each receipt shall contain all of the following information: <br /> -The name, address, identification number, contact person,and telephone number of the generator, and <br /> the signature of the generator or the generator's representative; <br /> -The date of the shipment; <br /> -The manifest number, <br /> -The volume or quantity of each waste stream received, its California and RCRA waste codes, the waste stream <br /> type listed in subdivision(c),and its proper shipping description, including the hazardous class and United <br /> Nations/North America(UN/NA)identification number, if applicable; <br /> -The name,address, and identification number of the authorized facility to which the hazardous waste will be <br /> transported; <br /> -The transporter's name, address,and identification number; <br /> -The driver's signature; <br /> -A statement, signed by the generator, certifying that the generator has established a program to reduce the <br /> volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator, to be <br /> economically practicable. <br /> Ensure that all future receipts for hazardous waste shipments using a consolidated manifesting procedure contain <br /> all required information. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by December 2, 2016. <br /> Please be aware as of January 1, 2013, all businesses are required to submit all hazardous materials <br /> information online to the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov. Be <br /> sure to include your hazardous waste activity in the Businesses Activities section in CERS in addition to any <br /> other relevant activities and required fields. <br /> Notes: <br /> -The facility generates used oil, used coolant, drained used oil filters, metal brake shavings used absorbent. <br /> -A white 55 gallon container of used coolant on the east side of the shop was open and not properly labeled. <br /> This was corrected on site. <br /> -Metal brake shavings on the south side of the facility not labeled and open.
