(continued) <br /> closure of the facility and shall include: <br /> - name &job title of each employee <br /> -written job description for each position Qob title) listed, including requisite skill, education, <br /> qualifications, and duties <br /> -description of training to be given to each position <br /> - records that training was completed for each employee <br /> Immediately prepare and implement a complete hazardous waste management training program for this <br /> facility in accordance with Title 22 regulations and ensure that all employees complete this training within <br /> six months of hire and at least annually thereafter. Submit a copy of the updated training program and <br /> records to the EHD by September 24, 2011. <br /> REPEAT VIOLATION <br /> 59. Manifest numbers 002595029FLE (6/16/09), 000850548GBF (7/28/09), 005991397JJK(9/30/09), <br /> 006069742JJK(3/1/10), 007083765JJK(4/23/10), and 008038569JJK(10/12/10)were found on site at <br /> the facility but were not on recons with the Department of Toxic Substances Control (DTSC). Within 30 <br /> days of each shipment, a generator shall submit a legible copy of each manifest used to DTSC. <br /> Immediately submit a copy of these manifests to DTSC and ensure that copies of all future manifests are <br /> submitted to DTSC within the required time frame. <br /> REPEAT VIOLATION <br /> 60, 61, & 62. Manifest numbers 0000771168GBF (10/18/08), 000850548GBF(7/28/09), 005991397JJK <br /> (9/30/09), 006998230JJK(4/19/10), 007427265JJK(9/14/10), and 007425891 JJK(4/19/11)were not <br /> found on site. Manifest numbers 002595029FLE(6/16/09), 007085492JJK (3/29/10), 007083765JJK <br /> (4/23/10), 007454462JJK(5/14/10), 007431033JJK(7/28/10), 007431034JJK (7/28/10), 008025694JJK <br /> (2/17/11), and 008388093JJK (5/27/11)were found without a signed off copy from the destination <br /> facility. Hazardous waste generators shall retain copies of all manifests signed off by the destination <br /> facility on site for three years and have them readily available for review. If the generator did not receive <br /> a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the <br /> generator's waste was submitted within 35 days of the date the waste was accepted by the initial <br /> transporter, the generator shall contact the transporter and/or the owner or operator of the designated <br /> facility to determine the status of the waste. If the generator did not receive a copy of the signed off <br /> copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter, the <br /> generator shall submit an Exception Report that includes a legible copy of the missing manifest, and a <br /> cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY I SUBJECT TO REINSPECTION AT ANY TIME AT EHIYS CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> EHD 23-02-003 Q..//412111 'CbITINUATION FORM <br />
