Below please find each completed corrective action in more detail. <br />1. Manifest Number 003629056SKS, "Designated Facility to Generator" form with the signature <br />of "Designated Facility Owner or Operator" was found. The manifest indicates the manifest <br />was received and signed on 3/12/13 and mailed on 3/18/13. — Photo enclosed. <br />2. The following Hazardous Waste Emergency Information has been updated and posted <br />onsite: - Photo enclosed. <br />• Emergency and Alternate Emergency Coordinator Information <br />• Locations of Fire Extinguishers <br />• Locations of Spill Control Equipment <br />• Phone numbers for Fire Department <br />3. Waste fluorescent bulbs have been placed inclosed containers and labeled with <br />"UNIVERSAL WASTE — LAMPS." Waste fluorescent bulbs will be recycled by Sprint's <br />recycling vendor shortly. - Photo enclosed. <br />4. The following Underground Storage Tank (UST) information has been updated/uploaded in <br />CERS: <br />• UST information, UST Monitoring Plan, Plot Plan. <br />5. Although the SPCC Plan onsite showed it was developed on November 5, 2008, it was last <br />reviewed by Sprint EHS personnel on August 26, 2011. This information is recorded in the <br />SPCC Plan's APPENDIX 3: SPCC PLAN REVIEW AND DOCUMENT LOG. — Photo <br />enclosed. <br />6. Documentation for unburied piping for emergency generator tank system and visual <br />inspections records are maintained in the SPCC binder. Refresher training records were <br />also maintained in the SPCC binder (under "Records" tab). <br />
