Insoection Report <br />Lawrence Livermore National Laboratories <br />Site 300 <br />Inspection date - October 28 and 29, 2003 <br />Mr. Shih that I would be going out to LLNLS300 on October 28, 2003 around 8:00 a.m.. <br />October 28, 2003 <br />I arrived at LLNLS300 badging office around 8:00 a.m.. I then phoned N1r. Shih so that he would know I was <br />on schedule. Mr. Shih arrived about the same time I did. We proceeded through the badging procedure while <br />we waited for Mr. Terusaki, and Ms. Folks. <br />After Mr. Terusaki and Ms. Folks arrived we proceeded to a conference room where we were joined by Mr. <br />Lane as well as other representative of LLNLS300. Introductions were made and a sign -in sheet was passed <br />around (Attachment F). <br />I began the in -briefing by explaining the purpose of my visit. I explained that the inspection would involve: <br />taking photographs, reviewing and copying records, questioning personnel and inspecting hazardous waste <br />handling areas. I also explained the role of the CUPA representative, Mr. Shih. I explained that while he was <br />here as an observer he could make note of any concerns he may have during the site inspection and discuss <br />it with them after my inspection had been completed. I then turned the meeting over to Mr. Anderson, who is <br />an Industrial Hygienist (IH) with the Department. Mr. Anderson explained his role in the inspection process. <br />He explained that it is the Department's policy that an IH accompany inspectors during inspections of <br />permitted facilities which have a permit to store mixed waste. He added that he would be conducting health <br />and safety monitoring using a Ludlum 19. 1 then asked for and received consent from Mr. Lane to conduct my <br />inspection. <br />I provided Ms. Folks with a printed list of records and documents and explained which ones I would need for <br />my review (Attachment N). We then discussed the areas which I planned to inspect. I explained that I would <br />be including generator areas as well as the permitted areas during this inspection. I added that I would <br />inspect the following areas: EWSF Bldg. 883, M-816, which includes M-1 and M-5, EWTF Bldg. 845-A/B <br />OD/OB, and EWTF Bldg. 829. 1 also explained that the generator locations I wished to inspect were the 90 - <br />day container storage area located in Bldg. 883 and generator storage in Bldg. 851. <br />BLDG. 883: <br />D,".> > n �� ^n <br />
