READ AND UNDERSTAND THIS WORK REQUEST PRIOR TO ARRIVING ON SITE Lori Duncan and client have been notified that OM will be on Mondav The scone of work will consist of i)urging and sampling four 2 -inch wells and collecting samples from three water su Iv wells two supply wells on site and one domestic well from neighbor to east Potential chemical hazards include gasoline constituents Items to bring to site include centrifugal pump and tubing bailers sample bailers water level probe and field meters ice chest with ice. 1-2 drums MONITORING WELLS Measure DTW/DTB in each site well DTW was approximately 6 5-7 5 feet below ground surface in June 2003 Order is MW4, MW3 MW2, MW1__ Purge and sample wells in order The order is MW4 MW3 MW2. MW 1 There may be drums on site You should only need 1 drum and you can combine all water into one drum Wells are 2 -inch diameter and approximately 20 feet bgs Measure water parameters - -a conductive tem Make sure you follow our decon protocols all downhole equipment between wells Sample all wells for TPHG/BTEX EPA 801513/8021B and oxygenates.
