See attached"Continuation-Official Inspection Report"for violation(s)not listed above. <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> ROUTINE HAZARDOUS WASTE GENERATOR INSPECTION <br /> Class II violations: (605, 703) <br /> 605. A 55 gallon white steel drum of acetone waste was observed in the chemical storage area with a <br /> blank hazardous waste label. All hazardous waste containers shall be marked with the following <br /> information: <br /> 1. the words "Hazardous Waste" <br /> 2. name and address of generator <br /> 3. hazardous properties (flammable, corrosive, reactive, toxic) <br /> 4. physical state (liquid, solid) <br /> 5. composition (contents) <br /> 6. accumulation start date <br /> Immediately label these containers and ensure that all containers are marked with all the required <br /> information. Since it cannot be determined how long the acetone waste has been on site, immediately <br /> contact a licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of <br /> the manifest to the EHD by May 3, 2012. <br /> REPEAT VIOLATION <br /> 703. At the time of inspection, it could not be demonstrated (not necessarily documented) that employees <br /> who handle hazardous waste were properly trained. Mr. Choopratheep could not recall the last time <br /> training had been conducted.
