Fill out this form completely and post next to all phones which would be used in an emergency. This will satisfy item#90 on the hazardous waste inspection from May 3, 07. Please send or drop off the manifests or disposal records at my office at 600 E Main Street. Also provide the "Return to Compliance Certification" and a statement which explains how the two items (#30 and#90) were corrected.
