EI Dorado, Stockton Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS I CLAS II or Ni1NOR-Notice to Com <br /> NOTES (The following are not violations) <br /> New owner began operating the referenced facility in December 2010. Facility generated less than 5 <br /> tons of hazardous waste in 2010- <br /> 52. An emergency coordinator and modified contingency plan information is lacking. There must be at <br /> least one emergency coorc inator on site or on call to coordinate emergency response measures, and <br /> the following information must be posted by a phone: the name and phone number of the emergency <br /> coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the <br /> phone number of the firedepartment, unless the facility has a direct alarm. Discrepancy was corrected <br /> on site. <br /> Discussed with owner the need to manage each type of Universal Wastes including aerosol cans, <br /> lamps, alkaline batteries, mercury containing switches, CRT, electronic devices, etc, in a closed <br /> container with a label identifying the content and the start date of accumulation_ Universal Wastes must <br /> be managed within 1 year of accumulation. <br /> Submit the following to EHD by : May 28, 2011 <br /> 1) A completed copy of the (provided) Return to Compliance (please keep original for your records). <br /> 2)A written statement how each violation was corrected <br /> 3) Evidence (i.e. photos, disposal receipts, documents) in support of the corrections made. <br /> ALL EHD STAFF TIME ASSociATED%WH FAILING TO COMPLY BY THE ABOVE NOTED DATES 1MLLL BE BO-LED AT THE CURREM HOURLY RATE($122). <br /> THIS FACILITY IS SU ECT TO REINSPECTION AT ANY TIME AT EHIYS CURRENT HOURLY RATE. <br /> EHD Inspects Received By: Title: <br /> ev <br /> IEHQ 2g oZ o03 RHOHo CONTINUATION FORM <br />
