Manifest numbers 017696761JJK (7/31 /2018) and 017468726JJK ( 10/24/2018) were found without a signed off copy from the destination facility. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those efforts . Exception reports shall be submitted to : DTSC Report Repository Generator Information Services Section P . O . Box 806 Sacramento , CA 95812-0806 Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC . Submit to the EHD a copy of the signed off copy of the manifests or a copy of the exception report submitted to DTSC . This is a Class II violation . 117 CCR 66265 . 16(a-c) Failed to complete training on hazardous waste management and emergency response procedures . The training described by the owner is incomplete . Personnel who handle hazardous waste shall be trained within six months of assignment and take part in an annual review of the initial training received . At a minimum , the training program shall be designed to ensure that facility personnel are able to respond effectively to emergencies by familiarizing them with emergency procedures , emergency equipment, and emergency systems, including where applicable : 1 . procedures for using , inspecting , repairing , and replacing facility emergency and monitoring equipment; 2, key parameters for automatic waste feed cut-off systems; 3, communications or alarm systems , 4, response to fires or explosions ; 5, response to ground-water contamination incidents ; and 6, shutdown of operations . The owner or operator shall maintain the following documents and records at the facility: 1 . the job title for each position at the facility related to hazardous waste management, and the name of each employee filling each job; 2 , written job description for each position listed above , 3 . written description of the type and amount of both introductory and continuing training that will be given to each person filing positions listed above , 4 , records that document that the training has been given to and completed by facility personnel . Training records on current employees shall be kept until closure of the facility. Training records on former employees shall be kept for at least three years from the date the employee last worked at the facility. Immediately provide employees with initial or refresher hazardous waste training . Submit a copy of the roster and the syllabus to the EHD . This is a Class II violation .
