The following is an itemized list of hazardous waste violations that have not been <br /> addressed for LODI OIL INC ARCO#83680 as of July 13, 2018. <br /> Open violations from May 30,2018 Inspection <br /> Violation#101 -Managed a hazardous waste without an identification number. <br /> This facility's hazardous waste ID number(CAL000402907)is inactive. A hazardous waste generator shall not <br /> treat, store, dispose of, transport or offer for transportation, hazardous waste without a hazardous waste ID number. <br /> Go to the DTSC web site http://www.dtsc.ca.gov/IDManifest/PERMHWID.cfm for the form to reactivate the ID <br /> number. Immediately reactivate your EPA ID number and submit evidence to the EHD. <br /> Violation#102-Failed to determine if a waste is a hazardous waste. <br /> According to Veer Singh(manager), after absorbent is placed on petroleum spills,the spent absorbent is then <br /> placed into the rock landscape area located in the back of the store. Any person who generates a waste shall <br /> determine if the waste is a hazardous waste. Immediately discontinue this practice and , make a hazardous waste <br /> determination for the fuel contaminated absorbent, and manage it according the Title 22 hazardous waste <br /> regulations. Submit a statement and supporting documentation explaining how this waste was managed. <br /> Violation#104-No modified contingency plan. <br /> An emergency coordinator and modified contingency plan information is lacking. There must be at least one <br /> emergency coordinator on site or on call to coordinate emergency response measures, and the following <br /> information must be posted by a phone:the name and phone number of the emergency coordinator; location of fire <br /> extinguishers, spill control equipment, and if present,fire alarm; and the phone number of the fire department, <br /> unless the facility has a direct alarm. Immediately appoint an emergency coordinator and post the required <br /> information by a phone. A form is provided that can be used for this purpose. <br /> Violation#106-Failed to train employees on waste handling and emergency procedures. <br /> At the time of inspection, it could not be demonstrated(not necessarily documented)that employees who handle <br /> hazardous waste were properly trained.Veer Singh(manager)explained that cleanup procedures involved placing <br /> clean absorbent onto spill, letting it soak,then placing the spent absorbent into the back rock landscaped area. The <br /> generator must ensure that all employees who handle hazardous waste are thoroughly familiar with proper waste <br /> handling and emergency procedures. Provide proof of training to the EHD for employees whose responsibilities <br /> include hazardous waste. <br /> Violation#301 -Facility not maintained to minimize the release of a hazardous waste. <br /> According to Veer Singh(manager), after spent absorbent is used to clean up fuel contaminated areas,the <br /> absorbent is poured into the rock landscaped area in the back of the store.Witnessed absorbent in the rock <br /> landscaped area located in the back of the store. Facilities shall be maintained and operated to minimize the <br /> possibility of a fire,explosion, or release of hazardous waste to air, soil, or surface water which could threaten <br /> human health or the environment. Immediately clean the absorbent, make a determination and manage according <br /> to Title 22 hazardous waste regulations.
