Bypassing of pretreatment facilities at any time is <br /> strictly prohibited without the express written consent <br /> of an authorized City of Stockton representative in <br /> advance. <br /> 6. Any breakdown or interruption of pretreatment, or any <br /> accidental discharge to the City of Stockton storm or <br /> sanitary system must be reported immediately. Non- <br /> reporting of spill or slug incidents will be cause for <br /> administrative permit review and/or- revocation of <br /> Permit. <br /> 7. Permittee is required to self-monitor wastewater <br /> generated onsite for pollutants specified by the City of <br /> Stockton. Sampling procedures and handling must be <br /> approved by the City of Stockton and the laboratory <br /> utilized must be certified by the State of California <br /> Department of Health- Services for the specific analysis <br /> conducted. Sampling frequency and reporting will be <br /> determi-ned on a "case-by- <br /> case" basis , but must at least comply with 40CFR 403. 12 <br /> (g) "Monitoring and Analysis to Demonstrate Continued <br /> Compliance" and any specific categorical requirements . <br /> S. If more than the required number of samples are taken <br /> for analysis, those results shall also be reported to <br /> the City of Stockton. Permitee must report all results <br /> (40 CFR 403.12(k)(1) . <br /> 9. All reports required will, at minimum, contain in <br /> addition to other requirements, acknowledgement of <br /> accidents of noncompliance and steps taken to correct <br /> the problem. All reports submitted to this Department <br /> must be signed by an officer or owner capable of binding <br /> the Permittee. <br /> 10. Annual and self-monitoring reports are required. An <br /> annual report shall be submitted as the annual <br /> Application Discharge, including all required <br /> attachments . Self-monitoring requirements and reporting <br /> are specific to the Sampling Schedule for each industry. <br /> Incidents of noncompliance and corrective action must be <br /> reported semiannually. <br /> 11. Permittee shall abide by all applicable provisions of <br /> the City of Stockton Municipal Code, or any applicable <br /> local, state or federal code or regulations ; and any <br /> �✓ continuing violation of any provision of said codes or <br /> regulations may be Just cause for revoking this Permit. <br /> Permit is non-transferable, and must be renewed every <br />
