Owners and operators of hazardous waste facilities shall have permits during the active life (including the closure period) of the facility and, for any facility which closes after January 26, 1983, during any post-closure care period required under Section 67217, and during an compliance period specified under Section 67186, including any extension of that compliance period under Section 67186 (c). (b) Permits are required for: (2) Treatment, storage or disposal of hazardous waste at facilities requiring an NPDES permit. The Biennial Report shall be submitted on Department forms and shall cover generator activities during the previous calendar year, and shall include the following information: (1) The EPA identification number, name and address of the generator; (2) The calendar years covered by the report; (3) The EPA identification number, name and address for each off-site treatment, storage or disposal facility to which waste was shipped during the year; for exported shipments, the report shall give the name and address of the foreign facility; (4) The name and EPA identification number of each transporter used during the reporting year; (5) A description, California hazardous waste category number, DOT hazard class and quantity of each hazardous waste shipped off site.
