Circuit Works, Inc. June 26, 1987 <br /> (a) Permits are required for treatment, storage or disposal of <br /> hazardous waste. Owners and operators of hazardous waste facilities shall <br /> have permits during the active life (including the closure period) of the <br /> facility and, for any facility which closes after January 26, 1983, during <br /> any post-closure care period required under Section 67217, and during an <br /> compliance period specified under Section 67186, including any extension of <br /> that compliance period under Section 67186 (c). <br /> (b) Permits are required for: <br /> (2) Treatment, storage or disposal of hazardous waste at facilities requiring <br /> an NPDES permit. <br /> CWI is operating a batch and pre-treatment facility without a <br /> permit from the Department. Two other treatment systems in place <br /> are trichloroethane recycling and chromic acid distillation. <br /> Violation #4 Section 66493 Title 22 California Administrative <br /> Code <br /> (a) A generator who ships his hazardous waste off site shall prepare <br /> and submit a single copy of a Biennial Report to the Department by March 1 of <br /> each even numbered year. The Biennial Report shall be submitted on <br /> Department forms and shall cover generator activities during the previous <br /> calendar year, and shall include the following information: <br /> (1) The EPA identification number, name and address of the generator; <br /> (2) The calendar years covered by the report; <br /> (3) The EPA identification number, name and address for each off-site <br /> treatment, storage or disposal facility to which waste was shipped during <br /> the year; for exported shipments, the report shall give the name and <br /> address of the foreign facility; <br /> (4) The name and EPA identification number of each transporter used <br /> during the reporting year; <br /> (5) A description, California hazardous waste category number, DOT <br /> hazard class and quantity of each hazardous waste shipped off site. This <br /> information shall be listed by EPA identification number of each off-site <br /> facility to which waste was shipped; <br /> 3 <br />
