This facility's EPA ID number is inactive. A hazardous waste generator shall not treat, store,
dispose of, transport or offer for transportation, hazardous waste without an EPA ID number.
Immediately contact DTSC and reactivate your EPA ID number and submit evidence to the EHD by
December 26, 2012. Norma showed me the questionnaire she had received from DTSC and stated she
will the number get it re-activated.
NOTES
Facility disposed no hazardous waste in 2011 or 2012 because business is so slow. Facility generates
hazardous waste from paint mix and occasionally metal waste mixed with bondo dust.
Observed one approximately 2/3 full 30-gallon drum of paint waste in a metal container with a label
lacking the accumulation start date. I reminded the generator that as a Conditionally Exempt Small
Quantity Generator (CESQG) which they claimed, the container must be labelled with the accumulation
start date once they reached 100 kilograms (220 pounds) of hazardous waste stored on site in total, not
just per container, and dispose of the waste in accordance to Title 22 within 3 months of that date.
Spoke to generator about CERS compliance requirement, and provided hand out & authorization form.
Complete and submit a copy of the Return to Compliance Certification form with a statement of how each
violation was corrected including supporting documentation by December 26 2012.
ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
THIS ACI ITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE
Inspecfi ecei BY Tice:
EH 2-02 &04961,21"" Page 4 HW INSPECTION REPORT
