• w .. <br /> The following is an itemized list of hazardous waste violations that have not been <br /> addressed for HIGH SPEED TRANSMISSION as of December31, 2015. <br /> Open violations from July 28, 2015 inspection <br /> Violation #403 - Failed to keep hazardous waste containers closed except when adding or removing <br /> hazardous waste. <br /> Found the following containers open: <br /> 1) 3-5 gallon containers of used oil <br /> 2) 2 blue rectangular metal container of used oil <br /> According to owner, Mike, the 3-5 gallon containers are emptied into the blue metal rectangular container weekly. All <br /> hazardous waste containers shall be closed at all times except when adding or removing waste. Immediately close <br /> these containers and ensure all hazardous waste containers are closed when not adding or removing waste. <br /> Violation#605- Failed to completely label containers or tanks of hazardous waste. <br /> Observed 2-25 gallon blue rectangular metal containers of used oil with the accumulation start date 5/21/14. Owner <br /> stated that there were disposal of waste oil after that date. Verified disposal records for waste oil and there were 9 <br /> disposal records after that date. All hazardous waste containers shall be marked with the following information: <br /> -the words"Hazardous Waste' <br /> -name and address of generator <br /> - hazardous properties (toxic, corrosive, flammable or reactive) <br /> - physical state <br /> -composition (contents) <br /> -accumulation start date <br /> Immediately label these containers and ensure that all containers are marked with all the required information. Verifie <br /> the last disposal record for used oil pickup on 5/19/15 and the owner will mark the accumulation start date as 5/19/1,1 <br /> on both blue rectangular metal containers since he did not have a marker to write legibly on the hazardous waste <br /> label. <br /> Page 1 of 1 <br />
