Grant St and are using the ID number for that address(CAL000205132). <br /> The correct ID number for this site is CAL000416532. The generator and waste sections of the uniform manifest <br /> shall be completed and signed and dated by the generator. The transporter shall also sign and date the manifest. <br /> Ensure that all manifests are properly completed for any transport of a hazardous waste for off-site transfer, <br /> treatment, storage or disposal. <br /> This is a minor violation. <br /> 113 HSC 25160.2(b)(3) Failed to keep copies of consolidated manifesting receipts for three years. <br /> Copies of hazardous waste disposal records for used oil for the past three years were not found on site. A single <br /> consolidated manifest was found for used oil dated 7/23/14. Hazardous waste generators shall retain copies of all <br /> manifests signed off by the disposal facility and all receipts used in a consolidated manifesting procedure on site for <br /> three years and have them readily available for review. Immediately locate a copy of all missing manifests and <br /> receipts for the last three years, maintain them on site, and submit copies to the EHD. <br /> This is a minor violation. <br /> 114 HSC 25160.2(b)(4) Consolidated manifest receipts failed to contain complete information. <br /> The consolidated manifest receipts contain incorrect information for the site address and the EPA ID number. They <br /> incorrectly list the address as 317 N. Grant St and are using the ID number for that address (CAL000205132). The <br /> correct ID number for this site is CAL000416532. All copies of each receipt shall contain all of the following <br /> information: <br /> -The name, address, identification number, contact person, and telephone number of the generator, and the <br /> signature of the generator or the generator's representative; <br /> -The date of the shipment; <br /> -The manifest number; <br /> -The volume or quantity of each waste stream received, its California and RCRA waste codes, the waste stream <br /> type listed in subdivision(c), and its proper shipping description, including the hazardous class and United <br /> Nations/North America (UN/NA) identification number, if applicable; <br /> -The name, address, and identification number of the authorized facility to which the hazardous waste will be <br /> transported; <br /> -The transporter's name, address, and identification number; <br /> -The driver's signature; <br /> -A statement, signed by the generator, certifying that the generator has established a program to reduce the <br /> volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator, to be <br /> economically practicable. <br /> Ensure that all future receipts for hazardous waste shipments using a consolidated manifesting procedure contain all <br /> required information. <br /> This is a minor violation. <br /> Page 4 of 5 <br />
