Contingency Plan/ CHP Emergency Action Plan is incomplete. The Plan states the incorrect phone <br /> number for San Joaquin County Office of Emergencies Services on page 1-1. On page 1-3, The Plan <br /> has a listing of Emergency Coordinators/Responders all whom are no longer working for this facility: Sgt. <br /> White, Nevins, Francone, Meaders. The Plan shall list names, addresses, phone numbers (office and <br /> home) of all persons qualified to act as emergency coordinators and the list shall be kept up to date. <br /> Correct by listing current Emergency Coordinators/Responders for this facility. <br /> 49. Personnel training records incomplete. The CHP Emergency Action Plan (EAP), training stated <br /> "review Emergency Action Plan annually". Reviewing the employee training log for each individual, <br /> some employees have never read the plan. Others have reviewed the Plan but the most recent date of <br /> review was from 2008 year. At minimum, training program shall be designed to ensure that facility <br /> personnel are able to respond effectively to emergencies by familiarizing themselves with emergency <br /> procedures, equipment, and hazardous waste management procedures. Ensure the employees are <br /> reviewing the EAP annually. <br /> 83. Failed to label universal waste. Two television monitors, one pink and one black, were found in the <br /> hallway leading from the office to the auto repair shop. It is unknown how long these were stored on-site. <br /> Once an electronic item is no longer in use or has no future use, the universal waste handler must <br /> clearly identify and mark these items as "Universal Waste - electronic devices". Correct by labeling these <br /> television with the above identification. <br /> 84. Storage of universal waste for unknown period. Two television monitors stored in the hallway have <br /> been stored for an unknown period of time. Universal waste can be stored on-site for up to one year of <br /> start accumulation date. Correct by properly disposing the two televisions to an authorized hazardous <br /> waste disposal facility. Retain disposal records onsite for a minimum of 3 years. <br /> 85. Failed to keep records of each shipment of universal waste. Per janitor, he has taken universal <br /> waste fluorescent tubes and waste aerosol cans to the Hazardous Waste Disposal facility located by the <br /> Stockton airport. Fluorescent tubes may contain mercury and aerosol may contain hazardous chemicals <br /> and propellant. No records or disposal receipts were available for review to ensure proper disposal of <br /> universal waste.
