Main Street <br /> Stockton, CA. 95202-3209 <br /> SUBJECT: Follow-up to Underground Storage Tank (UST) Program Inspection Report, <br /> Dated 9/2.6108, and Hazardous Waste Generator Program Inspection Report, Dated 9/26108, <br /> for City of Lodi Municipal Service Center, 1331 S. Ham Lane <br /> Your routine inspection of the City of Lodi's Underground Storage Tanks(UST's) and Annual <br /> Monitoring Certification noted three corrections that needed to be made. <br /> Enclosed please find the required follow-up paperwork showing compliance with your report's three <br /> items: Line #4, the Certification of Financial Responsibility and CFO Letter were, as noted, provided to <br /> you when you were on site-an additional copy of each is attached. Line#19, the Overfill Prevention <br /> System for Tank 3 -Diesel was set at 93% instead of 90% -as noted, this was corrected <br /> (reprogrammed) on site while you were present. Line#23, Designated Operator Monthly Inspection <br /> forms were noted as having inspection dates for the Monitoring Certification, Line Leak Detectors, and <br /> Spill Containment Testing as occurring on September 27, 2007-this is correct. The Dynamic Back <br /> Pressure Test and Static Pressure Decay Test were shown on one page as being done on <br /> September 28, 2007, while all other forms indicate work done on 9/27/07; Secondary Containment <br /> Testing of the entire Secondary Containment System was noted as done on October 5, 2007 (which <br /> was for the diesel only), when it should be November 8, 2005; the Designated Operator Monthly <br /> Inspection Forms have been corrected (11 copies attached). <br /> When you were on-site you also conducted a routine inspection of the City of Lodi's Hazardous Waste <br /> Generator Program and noted eight corrections that needed to be made. <br /> Enclosed please find the required follow-up paperwork showing compliance with your report's eight <br /> items: Line#16, that at the time of the inspection the secondary containment had liquid in it-the liquid <br /> andlor debris has been removed (see attached 2 photos). Line#20, that some containers of <br /> Hazardous Waste were not properly labeled-all containers of Hazardous Waste have been re-labeled <br /> with labels that include the following: 1)the words"Hazardous Waste", 2) the Generators name and <br /> address, 3) contents, 4) the physical state, 5) the hazardous properties and 6) the accumulation start <br /> date (see photos attached). Line#21, that one 55 gallon drum of SSIH was not closed-the drum is <br /> now closed and will be maintained closed unless adding or removing material. Line#24, that <br /> manifests were not properly completed, signed, or dated and Line#27, that manifests listed below were <br /> unavailable for viewing-all manifests for this facility are under the same Generator US EPA ID <br /> P:Ur 5JC EHO 102708.doc 10/2712008 <br />
