An emergency coordinator was missing. The contingency plan must <br />include: <br />1. Description of actions facility personnel will take in response to fires, explosions, or any sudden or <br />non -sudden release of hazardous waste to air, soil or surface water at the facility; <br />2. Description of arrangements made with local police departments, fire departments, hospitals, contractors and <br />State and local emergency response teams to coordinate emergency services; <br />3. List of names, addresses, and phone numbers (office/home/cell) of all persons qualified to act as emergency <br />coordinator. Where more than one person is listed, one shall be named as the primary emergency coordinator and <br />all others in order in which they will assume responsibilities; <br />4. List all emergency equipment, which includes location, physical description, and brief outline of its capabilities; <br />5. Evacuation routes for facility personnel where there is a possibility of evacuation; <br />6. Current telephone number for State Office of Emergency Services. <br />If the facility has a Spill Prevention, Control and Countermeasures plan, or some other emergency or contingency <br />plan, the owner or operator need only amend that plan to incorporate hazardous waste management provisions that <br />are sufficient to comply with the requirements of this chapter. Immediately prepare or amend an existing plan to <br />fulfill the requirements of this chapter. Submit a copy of the contingency plan to the EHD. This was corrected on <br />site. <br />This is a minor violation. <br />118 <br />CCR 66265.16(d) Failed to maintain complete personnel training records. <br />Employee training records for the management of hazardous waste were incomplete. The training records did not <br />have the list of employees trained. Employee training records shall include: <br />1. the job title for each position at the facility related to hazardous waste management, and the name of each <br />employee filling each job; <br />2. written job description for each position listed above; <br />3. written description of the type and amount of both introductory and continuing training that will be given to <br />each person filing positions listed above; <br />4. records that document that the training has been given to and completed by facility personnel. <br />Training records on current employees shall be kept until closure of the facility. Training records on former <br />employees shall be kept for at least three years from the date the employee last worked at the facility. Ensure that <br />employee training is properly documented. <br />This is a minor violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by December 9, 2016. <br />Please be aware as of January 1, 2013, all businesses are required to submit all hazardous materials <br />information online to the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov.
