A generator shall <br /> submit an exception report to the Department if the generator has not received a copy <br /> of the manifest with a handwritten signature of the owner or operator of the designated <br /> facility within 45 days of the date the waste was accepted by the initial transporter. <br /> Within 30 days, February 13, 2010 provide this office with TSDF copies of all of the <br /> above listed manifests or immediately submit Exception reports to the Department for <br /> the above listed manifests. <br /> 32) Failed to label oil filters containing gasoline/diesel commingled filters "used oil <br /> and gasoline filters": <br /> At time of inspection: <br /> 1) 1-55 gallon drum of diesel and oil filters was not labeled "used oil and gasoline <br /> filters" <br /> Containers of commingled filters must be labeled "used oil and gasoline filters". <br /> Immediately properly label all containers of commingled filters. <br /> 33) Failed to label Filter container with start accumulation date: <br /> At time of inspection: <br /> 1) 2-55 gallon drums of commingled gas and oil filters did not have a start <br /> accumulation date. <br /> Filter containers must be kept closed at all times unless actively adding or removing <br /> filters and must be labeled with a start accumulation date. Immediately date these <br /> containers with the start accumulation date. If it has been over 12 months, immediately <br /> have these filters removed under bill of lading and provide this office with a copy. <br /> 45) Contingency plan incomplete: <br /> At time of inspection this facility did not have a complete contingency plan. Immediately <br /> update the contingency plan to meet all requirements as listed in 66265.51. Within 30 <br /> days, February 13, 2010 provide this office with a copy of the facility's contingency plan. <br /> 49) Personnel training records incomplete: <br /> At time of inspection facility personnel training records could not be provided. Training <br /> shall be provided by a person trained in hazardous waste management procedures and <br /> shall include instruction which teaches facility personnel hazardous waste management <br /> procedures, including contingency plan implementation relevant to the positions in which <br /> they are employed. At a minimum training shall include emergency procedures, <br /> emergency equipment, and emergency systems, including where applicable: <br /> 1) Procedures for using, inspecting, repairing, and replacing facility emergency and <br /> monitoring equipment. <br /> 2) Key parameters for automatic waste feed cut off systems. <br /> 3) Communications or alarm systems <br /> 4) Response to fire or explosions <br /> 5) Response to ground water contamination incidents <br /> 6) Shutdown operations. <br /> Records must include: <br /> -Job Title for each position related to hazardous waste management at the facility and <br /> the name of the person in that position <br /> -Written job description for the above mentioned job titles <br /> EHD Inspecto 5 Received by: <br />
