0 0 <br />Corrective Actions / 4101 N Holly <br />Violation #4 <br />Disposed or caused disposal of hazardous waste at an unauthorized point. <br />Order <br />Immediately, cease cleaning equipment over sanitary sewer and any other unauthorized <br />disposal point, cease disposing of used oil filters at unauthorized locations, and prevent <br />oil from entering unauthorized locations. <br />Actions: <br />1 Place warning signs at sewer drain locations. ( Water Only No Oils or Other Materials <br />in this Drain) <br />2. Two minute training of all employees on proper waste oil disposal methods. <br />3. Two minute training of all employees on proper used oil filter disposal. <br />4. Two minute training of employees on proper equipment cleaning methods. <br />5. Placed plug in Boiler Room Drain. <br />Violation #10 <br />Failed to determine if a waste is a hazardous waste. <br />Order <br />Immediately, cease disposing of oily waste at unauthorized locations and repair leaking <br />equipment to prevent unauthorized disposal. <br />Actions: <br />See action 1 thru 5 above <br />6. Remove insulation material from thermal fluid piping and inspect for leaks and repair. <br />7. Preventative maintenance plan for prevention and detection of oil leaks Including how <br />the contaminated areas are to be cleaned. <br />Violation #12 <br />Facility not maintained to minimize the release of a hazardous waste. <br />8. Leaks at the piping joints of the asphalt storage tanks heating system were located and <br />repaired. Leaks at the mixing bucket heating system were located and repaired. <br />9. A daily inspection and check off sheet is implemented to assure that leaks are located <br />in a timely manner and repaired as soon as possible. <br />Violation 13, 14, and 17 <br />
