Box 198 tie X <br /> Lathrop, CA. 95330 <br /> Dear Bob Edson: <br /> REPORT OF VIOLATION AND SCHEDULE OF CORRECTION <br /> On December 1, 1989 Public Health Services-Environmental Health Division (PHS-EHD) <br /> conducted an inspection at the Simplot facility located at 16777 Howland Road in Lathrop. <br /> As a result of that inspection, violations of the hazardous waste statutes and or regulations <br /> were identified. Specific violations and required corrective actions are listed below. Failure <br /> to correct the identified violations within the schedule provided will result in PHS-EHD <br /> citing you for continuing/additional violations. <br /> I. REPORT OF VIOLATIONS <br /> A. Section 25250.8,Division 20 Health and Safety Code. <br /> At the time of inspection hazardous waste manifests for waste oil transported <br /> off site were not available for review. <br /> B. Section 67141 (d) Division 20 Health and Safety Code. <br /> At the time of inspection the contingency plan did not have the correct names, <br /> addresses and phone numbers for all persons qualified to act as emergency <br /> coordinators. <br /> Il. SCHEDULE OF CORRECTIONS <br /> Violation #1: Within 15 days of receipt of this report, provide copies of your <br /> waste oil manifests, or modified manifest to our office for <br /> review. <br /> A Division of San Joaquin County Health Care Services <br />
