Checklist provided on 06/16/11. <br /> Official Inspection Report presented on 06/20/11. <br /> Minor/ Notice to Comply violation#49. Repeat)Class II violation #15 <br /> 15. Failed to maintain spill control equipment. The drum storage container/secondary containment for <br /> the Way Oil and Soluble Oil, located by the outside of the Oil Shed, is filled with a couple inches of oil. <br /> All facility communication or alarm systems, fire protection equipment, spill control equipment, and <br /> decontamination equipment shall be tested and maintained as necessary to assure its proper operation <br /> in time of emergency. Correct immediately by removing the oil from the drum storage container. <br /> - Corrected on-site. It was discovered that one of the 55 gallon drums had a pin size hole. Employees <br /> removing leaked oil into used oil drum. <br /> 49. Personnel training records incomplete. Reviewing the log sheets of the the neutralization of liquid <br /> test solution reveals that more than one gallon of liquid is being neutralized at a time. Procedures states <br /> to neutralize no more than 1 gallon at a time. Laboratory employees last trained 05/2010 on acid /base <br /> neutralizing procedures. Employees must be trained in hazardous waste management procedures, <br /> emergency procedures, emergency equipment, emergency systems. Facility personnel shall take part in <br /> an annual review of initial training. Correct by immediately train the laboratory personnel in these <br /> procedures. Written training records must include: job title that is related to hazardous waste <br /> management, name of employee, job description, and description of training. <br /> Submit to SJC EHD within 30 days (07/18/2011): <br /> 1. one copy of the completed "Return to Compliance Certification" form <br /> 2. a written corrective action statement for each of the above violation <br /> 3. laboratory employee training records <br /> Notes: <br /> - In 2010 year, facility generated approximately 401 tons of hazardous waste. <br /> - Universal waste satellite accumulation in G&L (Garden & Lawn), E&I (Electronic & Instruments), Parts <br /> - Laboratory waste storage in Lab. Chemical Storage, under fume hoods of 2 lab rooms <br /> - Silver imaging waste in Print shop. - Paint or sandblast waste in Paint shop <br /> - Site under demolition/reconstruction.
