Response to Alleged Violations
#15: Failed to maintain spill control equipment.
This item was due to an accumulation of oil inside a secondary containment container. The container
had been inspected the prior week with no findings. After investigating the cause of the oil it was
determined that a "pin hole" leak was found in the bottom of the drum. The oil inside the drum was
transferred to a new properly labeled container and the secondary containment was cleaned and
residual oil properly disposed of.This item was corrected 6-16-2011 during the inspection.
#49: Personnel training record incomplete.
During a review of log sheets for the neutralization of test solutions in the lab it was discovered that
more than 1 gallon batches were being neutralized. The procedure states no more than 1 gallon at a
time shall be neutralized. Also, laboratory employees were last trained on 5-20-2010 on neutralization
procedures. Annual refresher training was completed for lab personnel with the exception of an
employee that is vacation. That employee will be retrained upon their return to work.Training records
have been attached.
*Items not listed on Field Inspection Reported but noted on Official Inspection Report:
Professionally Engineered SPCC Plan is not current.
• Emergency contact address for S!C OES is not current.
Address has been updated on SPCC Plan.
• Inconsistent tank capacity diesel fire pump tank.
Plan has been updated to reflect correct tank capacities on inventory and written plan.
• Plan states that gasoline and diesel fuel tank will be inspected every 10 years.
The tank was inspected per APCD permit requirements on 1-29-2010 and 11-18-05 for AST Static
pressure Performance TP 201.36.The plan will be updated to reflect testing methods and
frequencies.
