(Alpha <br /> Inc.) Specifically for the above site. All personnel participating in fieldwork at this site <br /> must be trained in the general and specific hazards unique to the site, and if applicable, <br /> meet recommended medical examination requirements. <br /> All personnel involved in the fieldwork at this site will be issued a copy of this (SSP) <br /> prior to entering this site.
