"Simple <br /> Green" surfactant applied by the Contractor to th`e tank, reportedly in an attempt <br /> to facilitate decontamination activity. Ia n <br /> • July 1990, Spill report submitted to PHS/EHD <br /> J1 V1 <br /> ' • June 1991, Responsible party is identified and, Woodward-Clyde begins <br /> Remedial Investigation.
