The manifest should have been completed with EPA ID# CAL000343837. All <br /> copies of each receipt shall contain all of the following information: <br /> -The name, address, identification number, contact person, and telephone number of the generator, and the <br /> signature of the generator or the generator's representative, <br /> -The date of the shipment; <br /> -The manifest number; <br /> -The volume or quantity of each waste stream received, its California and RCRA waste codes, the waste stream <br /> type listed in subdivision(c), and its proper shipping description, including the hazardous class and United <br /> Nations/North America (UN/NA) identification number, if applicable, <br /> -The name, address, and identification number of the authorized facility to which the hazardous waste will be <br /> transported; <br /> -The transporter's name, address, and identification number; <br /> -The driver's signature, <br /> -A statement, signed by the generator, certifying that the generator has established a program to reduce the volume <br /> or quantity and toxicity of the hazardous waste to the degree, as determined by the generator, to be economically <br /> practicable. <br /> Ensure that all future receipts for hazardous waste shipments using a consolidated manifesting procedure contain all <br /> required information. This is a repeat violation, Class Il. <br /> 601 CCR 66262.34(d) Stored hazardous waste onsite greater than 180 days. <br /> The used oil tank has an accumulation start date of 1-13-14. Jas Bains states that she had a recent used oil pick up in <br /> June 2014 of approximately 150 gallons and that there is a few hundred gallons of used oil in the used oil tank at this <br /> time. Facilities who generate less than 1000 kg of hazardous waste per month and do not exceed 6000 kg of waste <br /> stored on site at any time may store waste on site up to 180 days. Immediately contact a licensed hazardous waste <br /> hauler to dispose of this waste under manifest and submit a copy of the manifest to the EHD. This is a Class II <br /> violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 2-20-15. <br /> Consent to perform the inspection, take photos and collect samples was given by Jas Bains. <br /> This facility generated .5625 tons of waste in the last calendar year (Jan-Dec). <br /> Please be aware as of January 1, 2013, all businesses are required to submit all hazardous materials information <br /> online to the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov.
