Failed to store Universal Waste (UW) lamps in closed container, in manner that prevents
breakage & release to environment. Failed to immediately containerized any broken lamps or releases to
the environment. Failed to label UW to identify UW from new lamps. Unable to demonstrate the length of
time UW has been accumulated from date it became waste.
Used fluorescent bulbs were observed stored open, broken and uncontainerized in outdoors trash area
within the enclosed fence. UW lamps shall be contained in a closed container or package that is
structurally sound, adequate to prevent breakage, and compatible with the contents of the lamp.
Immediately containerize all universal waste fluorescent lamps/bulbs in accordance with Title 22
regulations.
The bulbs were observed without the words 'universal waste.' A generator shall label or mark UW with
the words 'Universal Waste' and the type of waste (batteries, mercury containing equipment, lamps/bulbs,
electronic devices, CRTs, CRT glass, aerosol cans). Immediately label these wastes and ensure that all
future UW are labeled with the required information.
A generator of UW (batteries, mercury containing equipment, lamps/bulbs, electronic devices, CRTs,
CRT glass, aerosol cans) shall accumulate universal waste for no longer than one year from the date the
waste was generated. Since it is unclear when the maintenance crew left the bulbs outdoors,
immediately dispose of these waste fluorescent tubes and submit a copy of the disposal record to the
EHD.
ALL END STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE
Ineperlor:

RamrveO By:
Title:
PI -In "-n2 R
411912
Page 4
HW INSPECTION REPOR'
